Fluoxetine ranked first for young people's depression in a 2020 network meta-analysis, on low certainty evidence
71 trials, 9,510 young people. Only fluoxetine, alone or with CBT, beat pill placebo or psychological controls. Most results were rated low or very low certainty, and a 2026 re-analysis traced part of the effect to one trial.
Top of the evidence hierarchy, independently funded. how we score evidence
Caveat on this rating: Most estimates rated low or very low certainty; a 2026 re-analysis found one small, unreliable trial drove much of fluoxetine's apparent advantage.
The 2020 Lancet Psychiatry network meta-analysis that many guidelines lean on for fluoxetine in under 18s, with the later re-analysis that qualifies it.
Significant findings
Zhou and colleagues compared 16 antidepressants, seven psychotherapies and five combinations for the acute treatment of depressive disorder in children and adolescents, searching published and unpublished trials to January 2019 (PROSPERO CRD42015020841). They included 71 trials with 9,510 participants, mostly with moderate to severe depression. "Only fluoxetine plus CBT and fluoxetine were significantly more effective than pill placebo or psychological controls (SMDs ranged from -1·73 to -0·51)". Fluoxetine plus CBT beat CBT alone and psychodynamic therapy "but not more effective than fluoxetine alone". "No pharmacotherapy alone was more effective than psychotherapy alone." On dropouts, fluoxetine and nefazodone did better than sertraline, imipramine and desipramine, and imipramine did worse than placebo.
The authors concluded that "Despite the scarcity of high-quality evidence, fluoxetine (alone or in combination with CBT) seems to be the best choice for the acute treatment of moderate-to-severe depressive disorder in children and adolescents."
The other direction
The same abstract says "Most of the results were rated as 'low' to 'very low' in terms of confidence of evidence". In 2026 Lyus and colleagues (in this library) showed that one small 2006 trial from Iran drove much of fluoxetine's advantage in this analysis and in Cipriani 2016: without it, the effect against placebo fell from 0.51 to 0.29, close to the 2021 Cochrane review's small and unimportant estimate (Hetrick 2021, also in this library). The authors themselves say effects "might vary between individuals" and that suicide risk must be weighed case by case. Funded by China's National Key Research and Development Program.
What this does not show
It does not show any antidepressant is a large help for most young people, and it does not compare treatment with no treatment over months. It is not a reason to start, stop or change a child's treatment.
Worth asking
For my child, what would we expect fluoxetine to change, how would we measure it, and what about therapy alongside it?

Questions
How strong is the evidence behind this?
veisund rates this source "gold standard". Top of the evidence hierarchy, independently funded. It scores 89 out of 100 on our published rubric. One caveat travels with that badge: Most estimates rated low or very low certainty; a 2026 re-analysis found one small, unreliable trial drove much of fluoxetine's apparent advantage. The score is calculated from recorded facts about the source — study design, funding, publication venue, sample size, preregistration — not typed in by an editor.

Who paid for this research, and does that matter?
Study design: Meta-analysis of randomised trials. Funding: Independently funded. Independence from the proponent is not recorded. Industry sponsorship is one of the most reliably measured biases in medicine, which is why funding carries real weight in the score rather than sitting in a footnote.
