did the fda add a boxed warning on complex sleep behaviors to the z-drugs in 2019?

verdict: confirmed
the primary source says it, at that magnitude
“In 2019, the FDA added a boxed warning to this class.”

first, the part that holds

this one holds up. in 2019 the fda required a boxed warning about complex sleep behaviors (sleepwalking, sleep-driving and doing other things while not fully awake) on all three z-drugs: zolpidem, eszopiclone and zaleplon.

what we found when we went and looked

we read the fda safety alert posted 04/30/2019. it names eszopiclone, zaleplon and zolpidem and says rare but serious injuries, including deaths, followed sleepwalking, sleep driving and other activities done while not fully awake. we then opened the 2019 fda-approved labels for one product of each drug. each has a new boxed warning titled "warning: complex sleep behaviors", and the eszopiclone and zolpidem labels list "boxed warning 08/2019" under recent major changes. the same labels also added a contraindication for people who have had a complex sleep behavior on the drug, which the claim leaves out.

assertion by assertion

assertionverdictwhat the record says
the action happened in 2019confirmedthe fda safety alert was posted 04/30/2019, and the labels we read show the boxed warning change dated 08/2019. the announcement and the label revisions both fall in 2019. [1][2][3][4]
the fda added a boxed warningconfirmedall three 2019 labels open with a boxed warning. the eszopiclone and zolpidem highlights list "boxed warning 08/2019" under recent major changes, which marks it as new, not carried over from an earlier label. [2][3][4]
the warning is about complex sleep behaviorsconfirmedthe box reads: "complex sleep behaviors including sleep-walking, sleep-driving, and engaging in other activities while not fully awake may occur... some of these events may result in serious injuries, including death." [1][2][3][4]
it applies to the z-drug class: zolpidem, eszopiclone and zaleplonconfirmedthe fda alert names exactly these three drugs, and we found the boxed warning on a 2019 label for each one. the fda listed the drugs by name rather than using the word "class". still, these three are the whole group the claim describes, so calling it a class is fair. [1][2][3][4]
the 2019 action also added a contraindication, which the claim leaves outundersoldeach label we read lists a new contraindication, dated 08/2019 on the eszopiclone and zolpidem labels, for "patients who have experienced complex sleep behaviors after taking" the drug. the claim mentions only the box, so the 2019 action was broader than stated. [2][3][4]

both directions

for the speaker: every clause matches the record: the year, the agency, the boxed format, the subject (complex sleep behaviors) and all three drugs. the fda alert is dated 04/30/2019, and the eszopiclone and zolpidem labels list the boxed warning as a recent major change dated 08/2019. the zaleplon label, revised 08/2019, has the same box.

against: nothing contradicts the claim. two small points of precision: the fda named three drugs rather than declaring a \"class\" warning, and the 2019 action also added a contraindication, so it went further than the claim says. listeners may also run the april announcement and the august label revisions together, but both are 2019.

what this cannot say

we read one 2019 label for each drug: eszopiclone, zaleplon, and a sublingual zolpidem product. we did not open every zolpidem product's label, though the fda alert covers all of them by name. the full fda drug safety communication, which gives the count of injury and death reports behind the decision, would not load for us. so we report no case or death numbers, only that the fda says deaths happened. the boxed warning reflects reports to the fda's adverse event system, and those reports cannot tell anyone how often these behaviors occur. we also did not trace how earlier, non-boxed label warnings about sleep behaviors changed over time.
